If your licence is held with the DVLA, we can complete the D4 medical examination report at the same appointment.

How it works
Tell us your licence and which form you have, and we will confirm your appointment at Greysteel.
Bring your form, photo ID, glasses and medication list. The assessment is carried out by our clinician.
We complete the medical sections where the standards are met, ready for you to submit to the DVA or DVLA.
