About this test

Is the Bone Health Blood Test right for you?

Who it’s for

  • You have risk factors for weak bones, such as an early menopause or a parent who broke a hip, and want to check for things that can affect bone health.
  • You may be low in vitamin D, for example because you get little sun or have darker skin.
  • You have a condition that affects how you absorb nutrients, such as coeliac disease.
  • A previous test showed a high or low calcium level, and you want to recheck it alongside parathyroid hormone.

What it can show

  • Adjusted calcium and parathyroid hormone (PTH) are looked at together. PTH controls the calcium level in your blood, and too much PTH (hyperparathyroidism) can affect your bones and kidneys.
  • Vitamin D helps control the amount of calcium and phosphate in your body. Between October and early March, the sun in the UK is not strong enough for your body to make enough of it.
  • Phosphate is a mineral that is kept in balance alongside calcium by vitamin D and PTH.
  • Alkaline phosphatase (ALP) comes mainly from the liver and bones. A raised level can come from either, and a raised ALP with low vitamin D suggests it is coming from bone.
  • Albumin is the main protein in your blood. It is used to work out your adjusted calcium, because some calcium is carried by albumin.

What it can’t tell you

  • Blood tests cannot measure bone strength or diagnose osteoporosis. That needs a bone density (DEXA) scan and a fracture risk assessment. Blood tests help look for conditions that can affect bones.
  • One result is a snapshot. NICE advises that a raised calcium result is repeated before further tests are considered.
  • ALP is normally higher during pregnancy and in teenagers who are still growing, and it can also rise because of the liver.
  • Results need to be interpreted alongside your symptoms and history. Diagnosis and treatment of bone conditions are with your GP or specialist.

Common questions

Questions about this test.

No. Osteoporosis is diagnosed with a bone density (DEXA) scan. Blood tests can help find conditions that weaken bones, such as low vitamin D or an overactive parathyroid gland.
