About this test

Is the Iron Status Blood Test right for you?

Who it’s for

  • You have symptoms that can be linked to low iron, such as tiredness, shortness of breath, heart palpitations, pale skin or restless legs.
  • You have heavy periods, give blood regularly or eat few iron-rich foods.
  • You have been treated for low iron and want to recheck your levels as agreed with your doctor.
  • A close family member has haemochromatosis, an inherited condition where iron builds up in the body, and you want to know your iron levels.

What it can show

  • Ferritin is the protein that stores iron. It is the most reliable blood test for your body’s iron stores, and a low ferritin suggests low iron stores.
  • Serum iron, transferrin and TIBC (total iron binding capacity) show how much iron is circulating and how much capacity your blood has to carry it. Transferrin saturation shows how much of that capacity is being used.
  • Together, the results can suggest low iron stores, or iron levels that are higher than expected and may need further investigation.

What it can’t tell you

  • This test does not include a full blood count, so it cannot show whether you have anaemia. The Anaemia Profile includes both.
  • Ferritin rises with infection, inflammation and liver disease. A normal or high ferritin does not always mean your iron stores are normal.
  • Serum iron on its own is not a reliable guide to iron stores. It is affected by inflammation, recent iron tablets and the contraceptive pill.
  • Ferritin is less reliable in the second and third trimesters of pregnancy, when it falls naturally.
  • Low iron is a finding, not a diagnosis of its cause. Results are interpreted alongside your symptoms, and finding and treating the cause is done with your GP or specialist.

Common questions

Questions about this test.

NICE guidance says a ferritin below 30 micrograms per litre indicates iron deficiency when it is interpreted as part of a full assessment. Infection and inflammation can raise ferritin, so a doctor reads it alongside your other results and symptoms.
