About this test

Is the Heart Health Blood Test right for you?

Who it’s for

  • You are over 40, or you have never had your cholesterol checked. High cholesterol does not usually cause symptoms, so a blood test is the only way to know.
  • High cholesterol, heart disease or stroke runs in your family, especially at a young age.
  • You have other risk factors for heart disease, such as high blood pressure, diabetes, being overweight or smoking.
  • You want a more detailed picture than a standard cholesterol test, including inherited markers such as lipoprotein(a).

What it can show

  • Total cholesterol, HDL (“good”) cholesterol, LDL cholesterol, triglycerides and the total cholesterol to HDL ratio make up a standard lipid profile. Doctors combine these with your age, blood pressure and other factors to estimate your risk of heart attack and stroke.
  • Lipoprotein(a), or Lp(a), is a cholesterol-carrying particle whose level is mostly set by your genes and changes little over your life. A high level can add to your risk of heart disease, especially when LDL cholesterol is also raised.
  • Apolipoprotein B is the main protein on LDL and other “bad” cholesterol particles, and apolipoprotein A-I is the main protein on HDL. A higher apo B to apo A-I ratio may point to a higher risk of heart disease.
  • Apolipoprotein E helps clear fats from the blood, and small LDL cholesterol measures the smaller, denser LDL particles. Both are specialist markers that can add detail to a lipid profile.
  • CRP and high-sensitivity CRP (hsCRP) measure inflammation. hsCRP picks up low levels of inflammation that have been linked with heart disease risk, while standard CRP shows the higher levels caused by infection or inflammatory conditions.

What it can’t tell you

  • UK guidance estimates heart risk using total and HDL cholesterol alongside age, blood pressure and other factors. Apolipoproteins, Lp(a), small LDL and hsCRP add detail, but they are not part of routine NHS risk assessment and their exact role is still being studied.
  • Cholesterol can be temporarily lower during or after an illness and is higher in pregnancy. Some medicines, such as steroids and some contraceptive pills, can also change lipid results.
  • CRP and hsCRP rise with any infection, injury or inflammation, so a raised result may have nothing to do with your heart.
  • Blood tests are only one part of your heart risk. Blood pressure, smoking, diabetes, weight and family history all matter too, so a normal result does not mean you have no risk.
  • Results need to be interpreted alongside your health and family history. Diagnosis and any treatment, such as cholesterol-lowering medicine, are decided with your GP or specialist.

Common questions

Questions about this test.

Usually not. The NHS says most people do not need to fast, and UK guidelines no longer require a fasting sample for a lipid profile. If your triglycerides come back very high, a doctor may ask you to repeat the test after fasting.
