About this test

Is the Advanced Screen Blood Test right for you?

Who it’s for

  • You want a detailed picture of your heart and metabolic health that goes beyond a standard cholesterol test.
  • Heart disease or stroke at a young age runs in your family and you want markers such as lipoprotein(a) checked.
  • You have ongoing symptoms such as tiredness and want thyroid, iron, kidney and liver markers checked together.
  • You want a very wide panel that also covers vitamins, hormones, gut and immune markers (Advanced Screen Plus).

What it can show

  • Heart markers go beyond a standard cholesterol test. As well as total, HDL and LDL cholesterol and triglycerides, it measures apolipoproteins, small LDL and lipoprotein(a), a mainly inherited cholesterol particle that is not usually measured in a routine cholesterol test.
  • Glucose, HbA1c, insulin, C-peptide and adiponectin can help show how your body is handling sugar, including signs of insulin resistance. Uric acid can be raised in gout.
  • Liver tests, kidney tests (creatinine, eGFR, cystatin C and a urine test), a full blood count, iron studies, ferritin, magnesium, adjusted calcium and vitamin D give a broad view of organ function, anaemia and nutrition. Thyroid tests (TSH, free T4, free T3 and antibodies) show how your thyroid is working.
  • Men also have a PSA test and women a CA 125 test. These are tumour markers, which can be raised by many conditions other than cancer and are not screening tests on their own.
  • Advanced Screen Plus adds vitamin B12, folate, PTH, creatine kinase (a muscle enzyme), pancreas enzymes, sex hormones, immune proteins (immunoglobulins, complement, rheumatoid factor and ASO), an H. pylori antibody test, a coeliac antibody test (tTG IgA) and an IgE allergy test. These can help look into symptoms such as tiredness, joint pain, indigestion or gut problems. It also reports total antioxidant status, which is not used in NHS or NICE guidance and is background information only.

What it can’t tell you

  • PSA and CA 125 are not screening tests for cancer on their own. Raised levels have many non-cancer causes, such as a urine infection for PSA or endometriosis, fibroids or pregnancy for CA 125, and a normal result does not rule cancer out.
  • PSA testing is an informed choice. It can help pick up prostate cancer early, but it can also miss cancer or lead to tests and treatment you may not need, which is why routine PSA screening is not offered in the UK or Ireland for most men.
  • Some Plus results need careful interpretation. A positive IgE allergy test shows sensitisation, not necessarily an allergy, the coeliac test is only accurate if you are eating gluten, and NICE recommends a breath test, not a blood test, to check whether H. pylori treatment has worked.
  • Several results vary from day to day. Hormones change with the time of day and menstrual cycle, vitamin D with the season, uric acid can be lower during a gout flare and creatine kinase rises after hard exercise, so some results may need repeating.
  • Results need to be interpreted alongside your symptoms and medical history. Diagnosis and treatment of any condition are made with your GP or specialist.
